Enhancing Maternal and Newborn Outcomes in Ghana: A Comprehensive Randomised Controlled Trial Evaluation of Obstetric Triage Effectiveness and Midwives Training

Despite notable progress in maternal and child healthcare, Ghana continues to face significant challenges, with maternal mortality rates at 263 per 100,000 live births and neonatal mortality rates at 23 per 1,000 live births – both well above global averages. These disparities are often attributed to limited adherence to best practices in clinical care and resource constraints in public health systems. In response, the Obstetric Triage Implementation Package (OTIP) was developed to address these needs and optimise existing healthcare resources.

This report presents the results from the baseline survey of a study investigating the impacts of OTIP (Obstetric Triage Implementation Package), as implemented at scale, as well as the impact of leader selection method, on service quality, health outcomes, knowledge, and team collaboration.

This report presents three key findings:

  • First, we obtained good overall balance across treatment arms in hospital, patient, and midwife characteristics.
  • Second, there is considerable scope for improvement in service delivery in the study hospitals.
  • Third, champions are significantly different from non-champions and different selection methods lead to different types of midwives being selected as champions. Being a champion is seen as desirable, with most midwives reporting that they would want to be considered for a similar position in the future.
